MHC Referral Guide

The Mercy Health Center Referral Guide is a tool for volunteers and staff to utilize to aid in integrating care for our patients. There are many functions to the application (some to roll out in the near future that have yet to be implemented). The primary purpose to the application is to educate those working at Mercy about the services within the organization , while placing an emphasis on connecting patients with those services. The bulk of the application is a Behavioral Health Risk and Physical Health Risk classification guide based on the Four Quadrant Clinical Integration Model. It is hypothesized that this tool will improve the access to services and decrease patients behavioral and physical health risks. Following the steps will result a "Quadrant Placement" for the patient, and the placement will show the suggested referrals and follow-up steps needed to usher the patient to the next step in Mercys Model of Care. Each classification guide is tailored to the users position at Mercy.
